I found this great rubric on a blog called Phys Ed Review. Great blog with lots of assessment resources, as well as some really cool PE Geek resources. And the guy is a huge Star Wars fan, so he must be awesome!
One of the standards in physical education is to promote lifelong fitness, and physical fitness outside of school. Would provide a great way for students to log activity minutes outside of school. Then being able to compare the days. Seeing which days have more or less active minutes, and then using cognitive skills to evaluate why that is. I found this on Pinterest.

Elementary P.E. Lesson plans. Some of these are pretty good; they're organized by different units and are PDF files so you can easily print them. There is a space on the l.p. where all the NASPE standards are listed and space to write in the state/ county standards.
