Pneumocystis pneumonia: Presentation:HIV+ male with cough and shortness or breath. There is hazy, predominantly perihilar mid and upper zone opacification with some interstitial prominence. A few discrete cysts (pneumatocoeles) measuring up to 1cm can be seen. No pleural effusion. No obvious nodal enlargement.

Pneumocystis pneumonia (PCP) is an atypical pulmonary infection and the most common opportunistic infection in patients with acquired immunodeficiency syndrome (AIDS). Histology demonstrates intralveolar eosinophylic masses with a foamy appearance, due to small cysts within which the pneumocystis jirovecii organism is found.

The Harmsen Lab is interested in immune responses unique to the lung. This includes immune-mediated lung damage in Pneumocystis pneumonia, the effects of lung infections on the development of asthma and the use of nanotechnology for delivery of anti-inflammatory drugs to the lungs.

This patient presents with symptoms and a chest X-ray consistent with Pneumocystis pneumonia (PCP) requiring Trimethoprim/Sulfamethoxazole (TMP-SMX) for treatment. Pneumocystis pneumonia is caused by Pneumocystis jiroveci (formerly known as Pneumocystis carinii), a yeast-like fungus, and is seen in patients with immunocompromise; particularly HIV and AIDS. It is a common infection leading to the diagnosis of HIV infection and up to 80% of patients with AIDS will get PCP at some time.

CXR and CT in immunosuppressed man w/ cough and SOB. CXR shows ill-defined hazy mid & lower zone opacification. CT shows ground glass change and interlobular septal thickening, giving a "crazy paving" pattern. No nodes or effusion. Diagnosis = pneumocystis pneumonia. #FOAMRad #FOAMed
