This is a maiko (apprentice geisha) about to become a geiko (geisha).  Teeth blackening during the heian era was done for cosmetic and health reasons. The teeth were protected more from decay stained, and it was more beautiful than having yellowing teeth with white face powder.  Now geisha do this as a part of important ceremonies if they wish.

Ohaguro is a Japanese aristocratic term pertaining to the custom of dyeing one’s teeth black. This custom existed from ancient times and was seen as beautiful. It was also a symbol of wealth and maturity among women until the end of the Meiji period
